Services & Membership

Acceptance of Terms

By booking appointments, purchasing services, or becoming a member of Precision Primary Care & Wellness ("Clinic," "we," "us," "our"), you ("patient," "member," "you") agree to these Terms and Conditions. If you do not agree, please do not use our services.

Services Provided

We provide primary care medical services and wellness therapies as described on our website. Services include but are not limited to: medical consultations, laboratory testing, genetic testing, IV nutrient therapy, red light therapy, microcurrent therapy, shockwave therapy, and body composition analysis (Menla scans).

Membership Plans

  • Core ($150/month): Includes unlimited office/telehealth visits, annual physical, basic labs, and baseline Menla scan

  • Deluxe ($250/month): Includes Core benefits plus enhanced testing, monthly wellness services, and member discounts (25% off wellness packages)

  • Premier ($375/month): Includes Deluxe benefits plus unlimited wellness services, advanced testing, and VIP concierge access (33% off wellness packages)

All membership fees are billed monthly on the date of enrollment. Members may cancel at any time with 90 days written notice. No refunds for partial months.

Wellness Service Packages

Wellness packages are valid for the specified time period (1-8 weeks for short-term; 3-6 months for long-term). Services must be used within the package validity period. Unused services expire and are non-refundable. Packages are non-transferable.

Payment & Appointments

Payment Terms

  • All fees must be paid at time of service or booking unless other arrangements are made

  • We accept cash, credit cards, debit cards, and HSA/FSA cards

  • Recurring monthly memberships will auto-charge your payment method on file

  • Failed payments may result in suspension of membership benefits until payment is received

  • A $35 fee applies to returned payments

  • Members are responsible for keeping payment information current

Cancellations and No-Shows

  • Appointments must be canceled at least 24 hours in advance

  • Late cancellations (less than 24 hours) will be charged a $50 fee- non Members only. 

  • No-shows will be charged the full appointment fee ($75-150 depending on service) Non Members only. 

  • Repeated no-shows may result in membership termination

Insurance

We are a cash-pay practice and do not bill insurance directly. We can provide superbills for potential reimbursement, but we make no guarantees regarding insurance coverage or reimbursement. You are responsible for all fees regardless of insurance status.

Legal Terms

Medical Services Disclaimer

Our services are not a substitute for emergency medical care. In case of emergency, call 911 immediately. We do not guarantee specific medical outcomes. Individual results may vary. All medical decisions are at the discretion of your physician.

Scope of Practice

Services are provided by licensed physicians and healthcare professionals in the State of Indiana. We comply with all Indiana medical licensing requirements under IC 25-22.5. Telemedicine services are provided only to patients physically located in Indiana at the time of service.

Modifications to Services

We reserve the right to modify service offerings, pricing, and membership benefits with 30 days notice to active members. Current members will be grandfathered at their existing pricing for 12 months from any price increase notification.

Liability Limitation

To the fullest extent permitted by Indiana law, our liability is limited to the amount you paid for the specific service in question. We are not liable for indirect, incidental, or consequential damages.

Governing Law

These Terms are governed by the laws of the State of Indiana. Any disputes will be resolved in the courts of Hancock County, Indiana.

Electronic Signature

Your electronic acceptance during online booking constitutes your legal signature and agreement to these policies, equivalent to a written signature under Indiana law (IC 26-2-8, Indiana Uniform Electronic Transactions Act).
