Refund Policy

Membership Refunds

  • 14-Day Money-Back Guarantee: First-time members can receive a full refund within 14 days of enrollment if not satisfied (one-time only, must be requested in writing)

  • Ongoing Members: No refunds for partial months. If you cancel, you will have access to benefits through the end of your current billing period

  • Annual Payments: If you pay annually, refunds are prorated based on unused months minus a $300 administrative fee

Wellness Package Refunds

  • 3-Day Window: Full refund available within 3 days of purchase if no services have been used

  • Partial Use: If you've started using services, refunds are calculated as: (Package Price - Value of Services Used - $100 administrative fee)

  • After 14 Days: No refunds available after 14 days from purchase date regardless of usage.

  • Individual Service Refunds

  • Services Rendered: Non-refundable once service is provided (IV therapy, red light, microcurrent, shockwave, consultations)

  • Lab Tests: Non-refundable once specimens are collected or tests are ordered

  • Genetic Tests: Non-refundable once specimen is submitted to laboratory

  • Refund Process

  • All refund requests must be submitted in writing via email to precision@precisionpcw.com or in person. Refunds are processed within 10 business days to the original payment method. Refunds cannot be issued as cash or to different payment methods.

Cancellation Policy

Membership Cancellations

  • Notice Required: 90 days written notice required to cancel membership

  • How to Cancel: Submit cancellation request via email to precision@precisionpcw.com or complete cancellation form in office

  • Access After Cancellation: You retain membership benefits through the end of your current paid billing period

  • Reactivation: If you rejoin within 6 months, you may resume at your previous membership tier and rate. After 6 months, current pricing applies

Appointment Cancellations

  • 24-Hour Notice: Appointments must be canceled at least 24 hours in advance to avoid fees for non-members.

  • How to Cancel: Call 317-642-0211, text  or cancel through patient portal or email at Precision@precisionpcw.com

  • Late Cancellation Fee: $50 for cancellations made less than 24 hours before appointment- non-Members only

  • No-Show Fee: Full appointment value ($75-150) charged for missed appointments without notice-Non-members only

  • Emergency Exceptions: Medical emergencies or hospitalizations exempt from cancellation fees (documentation required)

Wellness Package Cancellations

  • Within 3 Days: Full refund if no services used (see Refund Policy above)

  • After 3 Days: Packages cannot be canceled, but unused services remain valid through package expiration date

  • Expiration: Services not used within validity period (1-8 weeks or 3-6 months depending on package) expire with no refund

  • Extension Requests: Package extensions may be granted for medical emergencies (must be requested before expiration, documentation required)

Changes & Special Circumstances

Membership Changes

  • Upgrading: You may upgrade your membership tier at any time. Price difference is prorated for the current month

  • Downgrading: Downgrades take effect at the start of your next billing cycle (30 days notice recommended)

  • Pausing Membership: Members may pause membership for up to 3 months per year for medical reasons or extended travel (must provide documentation)

Service Modifications

  • Package Changes: Wellness packages cannot be modified once purchased. If needs change, consider purchasing an additional package

  • Service Substitutions: In rare cases where equipment is unavailable, we may substitute comparable services with your consent

  • Provider Changes: You may request a different provider at any time at no additional charge

Force Majeure

In the event of circumstances beyond our control (natural disasters, pandemics, government orders, facility emergencies), we may temporarily suspend services without liability. Membership fees may be prorated or credited for any period exceeding 14 days of closure.

Termination by Clinic

We reserve the right to terminate the patient relationship for:

  • Non-payment after 30 days

  • Repeated no-shows (3 or more within 6 months)

  • Abusive or threatening behavior toward staff

  • Violation of clinic policies

  • Non-compliance with treatment recommendations affecting safety

You will be notified in writing and provided with referrals to other providers.
