The 5 Systems That Determine How Long — and How Well — You Live

I spent more than a decade caring for patients in the hospital. Many arrived with what looked like a sudden health crisis: a heart problem, uncontrolled blood sugar, severe fatigue, a fall, or a condition that had finally become impossible to ignore. But the crisis was often only the moment the problem became visible. The biology behind it had usually been changing for years. That experience changed how I think about prevention. Good preventive care is not about ordering every available test or promising that one advanced panel can predict the future. It is about recognizing patterns early, using evidence-based screening, understanding risk, and paying attention when a patient's lived experience does not fit neatly inside the first set of results. I often hear some version of this: “I know something has changed, but I keep being told everything is normal.” Normal results are reassuring. They can rule out important problems and protect patients from unnecessary testing and treatment. But **normal does not always mean explained**. A test can be normal while fatigue, weight change, poor sleep, pain, brain fog, or reduced exercise tolerance still deserves a thoughtful evaluation. At Precision Primary Care & Wellness, I organize that evaluation around five connected systems: metabolic health, cardiovascular risk, hormonal health, inflammation and immune function, and sleep, recovery, and nervous-system health. These five systems do not replace routine primary care. They help us see the connections that a problem-by-problem approach can miss.



1. Metabolic Health: How Your Body Handles and Uses Energy

Metabolic health is bigger than the number on the scale. It includes how your body manages glucose, insulin, stored energy, muscle, liver health, blood pressure, and lipids. Fasting glucose and A1C are important, validated tools for identifying prediabetes and diabetes. They remain part of responsible screening. In some patients, however, they do not explain symptoms such as: - Energy crashes after meals - Increasing waist circumference - Difficulty losing weight despite consistent effort - Strong carbohydrate cravings - Elevated triglycerides or fatty liver risk - A strong family history of type 2 diabetes When the history and risk pattern warrant it, fasting insulin and a calculated estimate such as HOMA-IR may add context about insulin sensitivity. HOMA-IR is not a stand-alone diagnosis, and there is no single universal cutoff that applies to every patient or laboratory. It is one clue to interpret alongside glucose, A1C, lipids, blood pressure, medications, family history, body composition, and symptoms. The goal is not to label someone based on one additional number. It is to identify a modifiable pattern early enough to act on it. 2. Cardiovascular Health: More Than “Good” or “Bad” Cholesterol

A standard lipid panel matters. LDL cholesterol, HDL cholesterol, and triglycerides provide essential information, and LDL remains a major, treatable cardiovascular risk factor. But cholesterol should not be interpreted in isolation. Real cardiovascular risk also depends on age, blood pressure, diabetes, smoking, kidney disease, family history, physical activity, and other clinical factors. For some patients, additional tools can clarify the picture: -Lipoprotein(a), or Lp(a): a largely inherited risk factor that may be reasonable to measure at least once in adulthood - Apolipoprotein B, or ApoB: an estimate of the number of atherogenic lipoprotein particles, particularly useful in selected patients - High-sensitivity C-reactive protein, or hs-CRP: a possible risk-enhancing marker when interpreted in the right clinical setting - Coronary artery calcium, or CAC, scoring: an imaging tool that may help selected adults make a more informed decision about prevention


Not every patient needs every test. The right question is: Will this information change what we do?


Genetics belongs here as part of the context—not as destiny. Family history and inherited markers can show us where to pay closer attention, while lifestyle, medication, and ongoing preventive care remain powerful ways to reduce risk.

3. Hormonal Health: Listen to the Pattern, Not Just One Symptom

Hormonal symptoms can overlap with many other conditions. Fatigue, weight change, hair changes, disrupted sleep, low libido, mood changes, and loss of muscle can relate to thyroid disease, perimenopause or menopause, testosterone deficiency, medication effects, nutritional deficiency, sleep apnea, depression, chronic stress, or metabolic dysfunction. That overlap is exactly why the evaluation should be careful. For thyroid concerns, TSH is an appropriate and useful starting test for most patients. Depending on the history, examination, medication use, pregnancy status, family history, or previous results, free T4, thyroid antibodies, repeat testing, or thyroid imaging may be appropriate. Other times, the better medical decision is to look beyond the thyroid for the cause of the symptoms. Here is the distinction that matters: A normal TSH should not be treated as proof that your symptoms are imaginary. It means we have one useful piece of information—and we still need to decide what best explains the change you are experiencing. The same principle applies to sex hormones. Testing should be based on the clinical question and, when relevant, cycle timing, age, medications, and treatment goals. More data is not automatically better data. Thoughtful data is better.

4. Inflammation and Immune Function: A Clue, Not a Diagnosis

Inflammation is part of the body's normal defense and repair system. It becomes clinically important when it is excessive, persistent, or connected to an infection, autoimmune condition, tissue injury, metabolic disease, or another underlying process. There is no single “inflammation test” that explains every case. Markers such as CRP, hs-CRP, or ESR may be useful in the right setting, but they are nonspecific. A high result does not tell us the cause by itself, and a normal result does not exclude every inflammatory or autoimmune condition. That means symptoms and examination still matter. Depending on the patient, we may need to consider patterns such as: - Persistent joint pain, swelling, or morning stiffness - Recurrent fevers, rashes, or mouth ulcers - Ongoing digestive symptoms - Unexplained anemia or abnormal blood counts - Family history of autoimmune disease - Changes in exercise tolerance, recovery, or pain Targeted testing is more responsible than ordering a broad panel “just to see.” When a referral, imaging study, or specialist evaluation is the right next step, that should be part of the plan too.

5. Sleep, Recovery, and Nervous-System Health: The System That Changes Every Other System

Sleep and recovery are sometimes treated like lifestyle footnotes. Medically, they are not. Poor or fragmented sleep can affect appetite, blood pressure, glucose regulation, mood, pain sensitivity, concentration, and physical recovery. Chronic pain and stress can then make sleep worse, creating a cycle that is difficult to break without identifying the main driver. Obstructive sleep apnea is an important example. It does not only affect older men with obesity. Women may present with fatigue, insomnia, morning headaches, mood changes, or brain fog rather than dramatic snoring. A normal office oxygen level does not rule it out because the breathing disruption occurs during sleep. Depending on the history, a deeper evaluation may include: - Sleep duration, schedule, and sleep quality - Snoring, witnessed pauses, morning headaches, or daytime sleepiness - Medication, alcohol, caffeine, and pain patterns - Screening for insomnia, restless legs, anxiety, or depression - Home or laboratory sleep testing when clinically indicated - Recovery trends from a wearable, used as context rather than as a diagnosis Recovery is not laziness, and persistent exhaustion is not a character flaw. It is a signal worth understanding.

One is stopping the evaluation as soon as common screening results are normal. The other is ordering a long list of tests without a clear clinical question, then chasing every borderline result. Precision is built around a better middle ground.

Your symptoms are data. Testing will be thoughtful, not automatic—and the conversation will not end simply because the first result is normal."

Sometimes the right answer is additional laboratory testing. Sometimes it is imaging, a sleep study, medication review, a specialist referral, or a focused lifestyle intervention followed by reassessment. Sometimes the most responsible choice is not to order a requested test, and I will explain why and offer a reasonable next step instead of dismissing the concern. Patients should not have to argue to be heard. They should be able to participate in an informed medical decision.

Why the Five Systems Need to Be Read Together

These systems constantly influence one another: - Poor sleep can worsen appetite regulation and insulin sensitivity. - Metabolic dysfunction can increase cardiovascular risk. - Hormonal transitions can affect sleep, muscle, mood, and lipids. - Chronic pain or inflammation can interfere with movement and recovery. - Medications used for one problem can influence another system. This does not mean every symptom has one hidden “root cause.” Human biology is rarely that simple. It means a connected evaluation can help us prioritize the most likely explanations and choose the next step with the greatest potential value. Traditional primary care clinicians often want to do this work but practice inside a structure built around short, problem-focused visits. Do normal lab results mean I am healthy?

Normal results are reassuring and may rule out important conditions, but no set of labs can define your entire health. Results should be interpreted with your symptoms, examination, medications, family history, and risk factors.

Does Precision order every advanced test?

No. A test should answer a meaningful clinical question or have a reasonable chance of changing the plan. Precision uses advanced testing selectively—not as a substitute for a careful history, examination, or evidence-based screening. However, because we are not bound by insurance like traditional PCP, we are more inclined to practice common sense medicine.

What happens if my initial tests are normal but I still do not feel well?

We reassess. That can mean revisiting the timeline, examining sleep, nutrition, medications, mental health, pain, hormonal transitions, or other diagnoses; repeating or expanding testing when justified; or coordinating imaging and specialty care. A normal first result should guide the next decision, not end the conversation.
